Summary
As a key member of our team, you will direct the work responsible for the Product & Technology Team focused on designing an effective Population Health Management (PHM) system/application solution(s), with a focus on care planning and screening. You will lead a cross functional team to create compelling solutions that address the needs of patients and providers in the delivery of whole person care.
Your role in our mission
- Collaborate with stakeholders to understand the client’s business requirements.
- Set road map for Care Management Solution that realizes the highest value for the client. Collaborate with other leaders to ensure the roadmap is integrated into the larger PHM service roadmap, looking for synergies to create a more compelling experience.
- Define and design an effective Care Management and Screening solution that meets the needs of the larger PHM service, ensuring that it aligns with organizational goals and meets the client’s business requirements including price, scope, and time.
- Lead a cross-functional agile solution team(s) through the rituals of defining, designing, configurating, testing and delivering a compelling Care Planning solution.
- Conduct solution reviews and analyze for gaps between design and the client’s business requirements and provide guidance to eliminate the identified gaps.
- Serve as a consultant/thought-leader for PHM system/application solution(s).
- Provide subject matter expertise for care management and screening from both the clinical and member perspective.
- Collaborate with senior leaders, clinical and business stakeholders, business and technical architects.
- Understands limitations of the present technology and strategizes out-of-the-box concepts and directions to lead the company into the future.
- Support Gainwell and partner product teams in developing their product roadmaps when appropriate.
- Maintains awareness of trends and best practices for the industry and related technology fields to recommend and implement new technology processes as appropriate.
- Provide leadership and mentor less experienced employees.
What we're looking for
- Bachelor’s degree or equivalent combination of education and experience.
- A mix of clinical and business, engineering, information systems or related field preferred.
- Experience with care planning and developing use cases supporting population health management.
- 3-5 years of business analysis and re-engineering, business transformation, product management, financial analysis, system design/development or related experience.
- Experience working with company products and operating systems.
- Experience working with customer business processes.
- Experience planning and deploying clinical, business or IT initiatives.
- Effective communication and mentoring abilities, enabling collaboration with business and technology teams.
- Experience developing and using high-level models to collect, aggregate or disaggregate complex and conflicting information about the business.
What you should expect in this role
- This is a remote position with annual travel to Sacramento, CA.

The pay range for this position is $145,100 - $207,300 per year, however, the base pay offered may vary depending on geographic region, internal equity, job-related knowledge, skills, and experience among other factors.
