Summary

This is a key role within the Strategy and Stakeholder Engagement tower of the Population Health Management (PHM) project. The role will focus in the near term in California on engaging stakeholder groups and users of the PHM Service, a groundbreaking data platform for Medicaid that Gainwell has been selected to deliver, sponsored by the state of California. The ideal candidate has experience with managed care plans, population health management, and has familiarity working with the Department of Healthcare Services (DHCS) in California.

 

Your role in our mission

  • Develops deep understand of targeted stakeholders to identify key influencers and communication channels and methods to effectively engage with the stakeholder group.
  • Builds relationships and facilitates an on-going dialogue with the client and target stakeholder groups on PHM service and ways the service could better achieve its goals of improving Medi-Cal members’ health and experience by offering a whole-system, person-centered service.
  • Engages key stakeholders and stakeholder groups to surface unmet needs and map and document opportunities for the PHM service to meet those needs.
  • Provides input into developing and modifying systems to meet stakeholder needs and develops business specifications to support these modifications.
  • Facilitates meetings with stakeholder to gather and document requirements and explore potential solutions.
  • Participates in technical reviews and inspections to verify 'intent of change' is carried out through the entire project.
  • Creates and implements PHM service adoption strategies and plans for the stakeholder segment that identifies the highest value use cases of the PHM service and maximizes stakeholder adoption.
  • Develops recommendations to drive utilization and proficiency with the PHM Service, that increase benefit realization, and the achievement of results and outcomes tied to the wider PHM-CalAIM program.
  • Applies a structured methodology and brings a change management mindset to activities.
  • Supports communication efforts by designing and developing communication plans and writing as needed.
  • Assesses stakeholder buy in and support for the PHM service by conducting impact analysis and gathering direct feedback from stakeholder groups.
  • Defines and measures success metrics for the targeted stakeholder group.

 


What we're looking for

  • Bachelor's degree or equivalent combination of education and experience.
  • 5 years of working experience in managing large-scale change or transformation initiatives
  • Experience working with or at Managed Care Plans, with relevant knowledge of data, quality, and population health platforms
  • Demonstrated knowledge of the Department of Healthcare Services (DHCS) and their programs and initiatives for the Medi-Cal population.
  • Experience working with the interface of data and functional groups within an organization.
  • A collaborative approach to work, and the motivation to develop skills by taking ownership of new challenges.
  • Excellent presentation skills, and the ability to communicate complex technical concepts simply and effectively to all audiences.
  • Demonstrate the ability to conceptualize components of a business issue to aid in the analysis and definition of solutions.
  • Strong leadership skills and ability to work with a diverse audience.
  • Ability to take initiative and drive execution with minimal supervision.
  • Technical capabilities in business applications (Microsoft Office, Tableau) and healthcare applications.

 

What you should expect in this role

  • Onsite work in Sacramento, CA, or willingness to travel to Sacramento, CA, 20-30% time is required.
