Your role in our mission

  • Receive, manage and resolve referrals for Medi-Cal members’ case management services
  • Conduct assessments to identify Medi-Cal members’ care needs and preferences to develop and adjust comprehensive care plans tailored to their specific needs
  • Collaborate with a multi-disciplinary team to schedule, coordinate and monitor complex care plans involving dental and medical providers appointments, receipt of care, transportation, and language services
  • Record case information accurately and provide summaries when requested
  • Identify challenges related to case management and provide suggestions for improvement

What we're looking for

  • Proven work experience in case management with a preferred minimum of 3 years of experience in a case management position in the healthcare/insurance industry related to Medicaid and/or Medicare
  • Clinical and/or dental health experience is preferred
  • Professional or personal experience supporting individuals with disabilities, chronic illnesses, and/or experiencing homelessness preferred
  • Effective verbal and written communication skills, exceptional interpersonal, customer service, and problem-solving skills
  • Ability to build relationships with different types of people, including clients, caregivers, family members, community-based organizations, and members of the health care

What you should expect in this role

  • Hybrid work environment: On-site 2 days per week as needed for face-to-face and client meetings; remainder of the time is remote.
  • The office is located in Roseville, CA.
  • We are only considering California residents.
  • Full-time, 40 hours per week with a salaried compensation and full employee benefits package

 

The pay range for this position is $69,100.00 - $98,700.00 per year, however, the base pay offered may vary depending on geographic region, internal equity, job-related knowledge, skills, and experience among other factors.
